I had a smog check on an older car and it involved most of the time looking with mirrors everywhere including under the car. The car was failed due to an external nick on a intake boot (which did not leak into air intake when tested other places after the test). I was offered to have repairs done at the shop. The smog shop is trying to get unneeded repair business
